For the fillable sheet the perception skill does not add the +3, and the size bonus category gives autofills in for Armor and CMB as a benefit, and disadvangate, when it is actually good for one, but bad for the other. As an example a gnome would get a +1 to AC and CMB, when its size is really a negative for CMB/CMD
